Forms for current students

The following forms are central to every student’s INDS degree. In each case, the course indicated in parentheses to the right of the hyperlinked form indicates where it applies within the INDS curriculum. If you have any questions, please contact a member of INDS staff.

Course Change forms are only for those students who have approved degree plans that have been Docusigned by all mentors. Courses must be approved before the alternate course begins. You may not submit a Course Change Form after the first day of your final semester.

Step 1: Meet with your degree mentors, INDS faculty mentor and INDS advisor to discuss the changes you would like to make to your approved degree plan.

Step 2: Complete the Request for Changes to Approved Degree Plan form [pdf]

Step 3: Email the completed form to Holly Cudzilo. She will start the Docusign process to obtain approval and signatures.

INDS has limited funds to support students each academic year. Requests are accepted on a rolling basis and once funds have been depleted, the application will be closed for the remainder of that academic year. The maximum amount a student can receive over their INDS career is $500. INDS will only provide funding that can be paid for directly through University accounts. No reimbursements or cash advances will be approved.

Eligibility Rules

  1. Must be a declared INDS student.
  2. Must be currently enrolled in or have already completed INDS 335.
  3. Activity must be related to the student’s INDS degree.
  4. Be able to provide a thorough and detailed response to these questions:
    1. Explain the activity for which you are requesting the funds.
    2. How is this activity related to your INDS degree?
